The Good Shepherd Basketball CYO program 2025-26 Winter Season
Thank you for your interest in The Good Shepherd CYO Basketball program. There are few steps to take if you decide to register your child in the program. Data of your child is used by administers of the game to communicate and respond in daily activity and emergency situations. We are still accepting registration at this time for all grade levels.
1) Are you eligible to play? Confirm your child is eligible by clicking on the link "CYO Eligibility"
2) Submit the following forms online:
1) Registration – Health Authorization
2) Parental Permission and Acknowledgement/Code of Conduct
Each child must be submitted separately. Input data should be complete and with the correct information. There
is a submit button at the bottom of each page that must be clicked in order for the data to be accepted in the
system. After submission a window appears allowing you to view input data and print a copy of the submitted form. Registration is due by September 20, 2025.
3) Print and turn in the concussion and code of conduct/media forms and complete with
signatures.
4) Pay for your child(ren) to participate by Sept. 20, 2025 before practices starts. Your player will not be able to practice until all forms and full payment are received. Pay for your child(ren) uniform if needed at the time of ordering. No jersery will be ordered if payment is not received. Lead time for uniformed to be ordered is six weeks. League orders Uniforms twice in September in order to have them back for first day of league play on November 2, 2025.

Uniform cost not included in registration fee - uniform cost is $100 per set; $60 for individual pieces.

When first entering the CYO Basketball program a copy of the birth certificate for each child must be on file. This copy will be used until the child is no longer in our program and then destroyed.
