The Good Shepherd Basketball CYO program 2026-27 Winter Season
Thank you for your interest in The Good Shepherd CYO Basketball program. There are a few steps to take if you decide to register your child in the program. Data about your child is used by the game's administrators to communicate and respond during daily activities and emergency situations. We are accepting registrations at this time for all grade levels (2nd through 8th; 2nd plays at the 3rd-grade level).
1) Are you eligible to play? Confirm your child is eligible by clicking on the link below "CYO Eligibility"
2) In the registration system, you will be required to submit the following forms online (most forms are short):
A) General Registration Form
B) Program Eligibility
C) Uniform Selection
D) Emergency Contact
E) Health and Medical Information
F) Parental Permission and Release Form/Code of Conduct
G) Document Upload
H) Review and Payment
Each child must be submitted separately. If you are registering more than one player, add the first player to the cart, then register any remaining players, and check out to pay all at once. Input data should be complete and with the correct information. After submission, a window opens that lets you view the input data and print a copy of the submitted form. Registration deadline is September 25, 2026.
3) Pay for your child(ren) to participate by Sept. 25, 2026. Your player will not be able to practice until all forms and full payment are received. Pay for your child(ren)'s uniform if needed at the time of ordering. No jersey will be ordered if payment is not received. The lead time for ordering uniforms is 6 weeks. League orders uniforms twice in September in order to have them back by the first day of league play on October 31st, 2026.

3) Submit cash payment at uniform fittings (you will have to register online after submitting payment)
1 Player Participation - 3rd Grade Only ($225), 4th through 8th Grades ($280)
2 Player Participation ($500), 3 Player Participation ($725)
Uniform cost not included in registration fee - uniform cost is $85 per set; $50 for individual pieces
Electronic payment is preferred; if paying by check, they should be made out to: Church of the Good Shepherd CYO.
You can drop them off or mail to the church.

To participate in the CYO Basketball program, a copy of each child's birth certificate must be uploaded to the registration system, even if the child has played in prior years. This copy will be used until the child is no longer in our program and then deleted. You should not have to upload the birth certificate in future years. You will also need to upload proof of residency to show that you are within the boundaries in order to play for Good Shepherd.
